​The Referral Process

Young Person Led: We strongly encourage the young person to complete the form themselves (with parent/guardian support if needed).

Duration: The form takes approximately 15 minutes to complete. This helps the young person familiarise themselves with our approach and feel "at home" with the service.

Next Steps: Once submitted, our Care Coordinators will process the referral and send an SMS to the young person to schedule their initial appointment.
