Book your first appointment, where we'll find out if therapy is the right choice for you

It may be helpful to prepare some information before signing up.
We will need to ask you for:

Your personal details (name, age, and address)

Your Vitality membership details (membership number, OTP)

Your parents/carers details (full name, email)

Your contact information (email and mobile number)

An emergency contact (name, phone number). This person must be over the age of 18

Sign-up should be completed by the person seeking support. ​
​
If you are under 12 years old, you should complete the assessment with the help of a parent, guardian, or adult you trust.
